The following forms and documents must be submitted for the child’s application to be accepted.

FORMS – can be found on the website to be printed, completed and submitted

• Charter School Enrollment Application
• Legal Guardianship Form
• Release of Records Form
• Notification of Offense Form
• Home Facilitator Contract
• Instructional Property Contract
• Medical and Dental Forms

DOCUMENTATION – to be provided by the parent

• Copy of student Birth Certificate
• School Records
  • K-9th : Report cards from previous year of school and any reports the school can provide
  • 9th  (transferring) – 12th: official transcripts from previous school

• Proof of residence – utility bill, internet bill, lease or mortgage agreement, any legal or government letters
